Thank you Fierce Healthcare and Heather Landi for covering the expansion of Next Chapter Care, our virtual survivorship program. Through Next Chapter Care, members get dedicated nurse practitioner-led visits focused on medication adherence, mental and emotional health support, cancer surveillance, symptom management, and lifestyle goals, along with wellness education, peer support connections, and flexible engagement options. Led by Dr. Asma Dilawari, our medical director of oncology care, the program reflects what she saw firsthand in clinical practice. Patients finish treatment, hear "congratulations," and get sent on their way, only to return with a whole new set of challenges that are hard to address alone. "Some people have had to go through a major surgery, months of chemotherapy, and weeks of radiation," said Dr. Asma Dilawari, medical oncologist and medical director at Thyme Care. "It's no surprise that things don't simply go back to how they were before a cancer diagnosis just because those treatments are finished. People still need whole-person care, and that support needs to continue to evolve to meet their needs. Next Chapter Care was built for that level of continued care." Today we expanded Next Chapter Care, our virtual survivorship program, to more than 15,000 members nationwide. The moment an individual finishes treatment and transitions to recovery, is often the moment they feel the most unsupported and alone. Suddenly, they’ve gone from multiple visits with their Care Team every week to a single visit and a printed care plan once treatment is done. Next Chapter Care was built differently, extending the relationship our care teams already have with members through diagnosis and treatment into the months and years that follow. What's included: 🩺 Nurse-led survivorship visits covering symptom management, emotional health support, medication adherence, and ongoing healthcare, wellness, and lifestyle maintenance 📋 Personalized survivorship plans and actionable care summaries outlining treatment history and future care recommendations 💬 Lifestyle and wellness education, and connection to peer support groups 📱 Flexible follow-up through phone-based support and Thyme Care Connect, our self-service platform More than 18 million Americans are living after a cancer diagnosis. They deserve a care model built for the full arc of that experience, not just the treatment window. Next Chapter Care fills that gap. Read more: https://lnkd.in/gTcFuvAY

We’re meeting with health plan leaders at #AHIP26 to talk about building an oncology strategy designed for today's pressures. What's missing from yours? Let's find time to connect about it in Las Vegas:

"When a Care Partner hears someone is without food, they find a solution. When a Social Worker hears it, they ask why." That distinction, between solving the immediate need and addressing the root cause, is critical in oncology care. By combining care partners with licensed social workers, Thyme Care’s model delivers both rapid support and long-term stability for patients navigating complex challenges. For providers, this means fewer recurring barriers, more consistent engagement, and a care plan that can actually hold. Learn how this model supports better outcomes: https://hubs.li/Q04c8d1v0

Thyme Care reposted this
Yesterday, Thyme Care was named #18 on CNBC's 2026 #Disruptor50. I joined Julia Boorstin on CNBC to talk about it, one week after my wife Katy Shah and I welcomed our second child, Miki, into the world. Personally and professionally, a week I won't forget. We started Thyme Care six years ago, and today we have 120,000+ individuals with cancer in our care. 8 million Americans are able to reach us. I'm proud of the scale, because scale means more people getting real support during the hardest moment of their lives. But the best part of it all is the team I get to do it with and everyone who chooses to make Thyme Care their team. I've worked in oncology for 17 years. I have never been part of a team like this, and I'm energized by what we'll build together in the years to come. https://lnkd.in/e9ZVCvDy

Today, Thyme Care was named to CNBC's 2026 #Disruptor50. Two million Americans will be diagnosed with cancer this year. The system meant to support them through this experience, doesn't. That's the problem we set out to solve six years ago. Cancer isn't just a medical diagnosis. It's clinical, emotional, social, financial. It shows up in every part of a person's life, and every part of their family's life. We believed there was a better way to support the whole person across the whole journey. Over the past six years, we built a different model to close the gaps in cancer care. → A 500-person Care Team of oncology-trained doctors, nurses, social workers, and care partners that can act on those insights and support our members 24/7 → Direct integration with our members' treating oncologists → Partnerships with the country's largest health plans → A technology and AI platform that knows in real time who needs help and when For the first time, all four pieces are working together at scale. Being named to this list signals what the people inside Thyme Care already see every day: we're building the future of cancer care, and the country is taking notice. TIME just put words to something we hear from cancer patients every day. A patient, weeks into messaging her care team through a portal, finally wrote: "I just wanted you to hear my voice. I want you to hear me as a person." That's not a failure of compassion. It's a failure of design. Our own Stephanie Broadnax Broussard DSW, LCSW-S, APHSW-C, Director of Social Work at Thyme Care, is quoted in the piece: "I don't think it's that doctors don't want to talk to their patients. They want to be available. But in a fee-for-service world where they're asked to see so many patients a day and maneuver so quickly, it can be very difficult for them to get on the phone." For healthcare leaders, you may see this in your data. The ED visits, the late-stage escalations, the members who fell through the cracks because a question didn't get answered in time. That's why we've built the connective tissue it's missing with oncology-trained, fully staffed Care Team members available 24/7, working alongside treating oncologists so nothing falls through.
